 it says that the .initial is set to .threshold by default, but in the
 source:

 cache.org/trac/browser/bin/varnishd/cache/cache_backend_poll.c#L465]

 .initial is set to .threshold - 1, meaning that all backends start off as
 sick, and require one health check to become healthy.
